APPLICATION CHECK LIST

PART 1 - APPLICATION FOR ADMISSION
Complete the form.

Residence: on campus or off?
Columbia has learned that one of the very profound learning dynamics at College is residential life. The interaction with other students, as well as staff and student leaders, open doors for personal growth. All single, fulltime students under the age of 20 are expected to live in residences. Parents/Guardians with students under the age of 20 with extraordinary or unique circumstances must make a written appeal for special permission to an admissions counsellor. The letter must be included with the student's application and must include reasons for the exemption request.


PART 2 - AUTOBIOGRAPHICAL SKETCH
Write a brief autobiographical sketch. Questions are provided to guide you through this process. Please answer each question.

Personal Medical Health Information. Answer in as much detail as possible.
International Students should ensure that they have a minimum of 3 months of health insurance prior to arrival in Canada. After 3 months, students are eligible for BC Medical Coverage at a nominal fee. Students have the option of remaining on an existing plan, especially if covered by their parent's medical plan. You should check with your current insurance carrier prior to departure to ensure you have adequate coverage.

PART 3 - RESPONSIBILITIES OF MEMBERSHIP AND REFERENCE WAIVER FORM
Read carefully, sign and return with application. You application cannotbe processed without it signed and returned.
Please fill out your reference information and sign the waiver form.

2 PERSONAL REFERENCES
These references are confidential, and will be kept in your student file at the College. They are not intended to be read by you, but are to assist us in assessing your goals and needs as well as your gifts and interests. Please have your references download the form or fill it out online (links are below).

HIGH SCHOOL RECORDS
Make arrangements to have an official high school transcript sent to Admissions. Photocopies cannot be accepted. If you are enrolled in courses when applying, please have your high school send interim/midterm grades from your Grade 12 year.

POST-SECONDARY TRANSCRIPTS
If you have been enrolled in academic work beyond high school graduation, arrange to have your post-secondary transcripts sent to the Admissions Office. Photocopies cannot be accepted. If you are enrolled in courses when applying to Columbia Bible College, please submit a transcript that shows grades completed and courses in which you are currently enrolled. Our Registrar will then be able to do a transfer analysis for you.

□ A DIGITAL PHOTO (HEADSHOT) OF YOURSELF
Include with your application, a recent passport sized headshot picture of yourself via email to admissions@columbiabc.edu or directly to your Admissions Counsellor. We appreciate putting a face to your application. We will also use this picture for your student ID card so that you have it upon arrival.

APPLICATION FEES
Canadian and US Applicants: Application fee of $50 in Canadian funds.
International Applicants (non-US): Application fee of $450 in Canadian funds. $300 go towards your student fees as a deposit.
You may pay your application fee over the phone (1-800-283-0881) with Visa or MasterCard or you may mail in a cheque or money order. If you are an international student and need to use a bank wire, please contact us for detailed information.

Application Deadlines and Early Admission
Early Admission/Financial Aid Application Deadline (Fall): May 31st
Application Deadline (Fall): August 15th
Application Deadline for non-US Internationals (Fall): June 1st
Early Admission/Financial Aid Deadline (Spring): November 30th
Application Deadline (Spring): December 10th
Application Deadline for non-US Internationals (Spring): November 1st

• Acceptance will be extended once all forms have been received, including references and transcripts. Final acceptance notice will be given via phone or email along with a mailed acceptance letter and package.

• If final transcripts from high school are unavailable at the time of application, please forward a copy of your interim report and a conditional acceptance would be issued based on completion of Grade 12.

• Upon acceptance, resident students must submit a deposit to confirm dorm space and course registration. Commuter students must submit a deposit to confirm course registration. These fees go towards your cost of studying at Columbia Bible College. Deposit details will be sent with your acceptance package. Deposits are due by July 15 for Fall entry and November 30 for Spring entry.
