Refund Appeal Form - to be completed by students appealing refunds. Take the completed form to Reception to be signed. If there are questions about the form, please refer to our Refund Policy.

Waiver Form - to be filled out by each student at the beginning of each year at Columbia Bible College.

Enrolment Contracts - to be filled out by each student for their individual programs at registration each academic year.

Pre-Registration Forms - to be filled out by each student when paying their deposit for the following semester.

Fall Semester - Commuter Form
Fall Semester - Resident Form

Winter Semester - Commuter Form
Winter Semester - Resident Form

Parking Registration Form - to be filled out either at the beginning of the school year or the beginning of a semester, depending on when you begin your time at CBC.
